Before scheduling an intake appointment, I will do a free initial phone consultation that will last 15-20 minutes. During this conversation, we will discuss your reasons for seeking therapy and determine whether I can offer you the services that you need.
After the phone consultation, we will schedule the initial visit. During this appointment, which is called an intake session, I will ask you questions about what brings you to therapy, and I will learn more about your history. We will take the time to get to know each other better, and I will answer any questions that you may have about the experience.

I have placed my intake forms below, so that you may download, read, and fill them out at your convenience. Please print and complete them prior to your initial visit and bring them with you to your appointment, to ensure that we are able to make the most of our time together.
The following form provides information about expectations regarding treatment and privacy of treatment records. Please read the entire form carefully, but you are only required to print and bring in the signature page.
Form of Payment
You’ll also need to bring a form of payment, as payment is due at the time services are rendered. Payment can be made with cash, check, or debit card via Square Cash. Take a look at my FAQs for more information about payment and session fees.
